For another view on prostate cancer screening…

Right now, right around the country, well-meaning community groups, including men’s health groups, are out on the hustings, urging all men of a certain age to be screened for prostate cancer.

There are many potential downsides to such simplistic messages.

    “Early detection of prostate cancer has always been a strong public health message, but does this end-up causing more harm than good”? Apparently so, to the treasury coffers on Capital Hill. Notorious for the razor-gang treacherous tactics against the vulnerable men, old and infirm. Whatever it takes to abolish Medicare rebated PSA testing altogether. Having learned hard-way apropos as a result of the fly-by-night operators sabotaged certain bulk-billed blood pathology tests, we know nothing about. Medicare targeted PSA screening in collusion with the Pathology Corporations run blood collection centres. July 20, 2009 edition of The Australian informed its dedicated readers that:

    “Private pathology corporations are seeking doctors’ help to mount a mass retreat from bulk-billing. A move expected to land many patients with out-of-pocket costs averaging $40, or up to $100 in some cases, for tests they typically now receive for free”! “Member companies from each of the three big corporations that dominate the pathology market have recently written to doctors who order tests, asking that in future they only ask the laboratory to bulk-bill a patient if the doctor believes the patient to be ‘financially disadvantaged'”. “The Australian has obtained similar letters sent by Gribbles Pathology, part of the Healthscope group, Sullivan Nicolaides Pathology which like Douglass Hanly Moir is part of the Sonic group, plus the Laverty Pathology and QML Pathology, both part of Symbion Pathology”!

    Except for the principled Coastal Pathology (the largest independently owned medical testing laboratory on the Sunshine Coast) which announced on Aug 2009: “In recent times, our competitors have made major changes to their billing practices, with variable charging of private fees for pathology services, in contrast to the previous longstanding, widespread practice of bulk-billing for outpatient services. As a medical practitioner, you may (or may not) have received notification of this change. As a patient, you may have received an unexpected bill following a blood test. Rest assured: Coastal Pathology has no plans to follow the path of our competitors.     Yet at times even physically fit sportsmen, parliamentarians like Wayne Swan, or the high profile younger men revealed in public their battle with the prostate cancer, while stressing importance in keeping-up with the regular PSA screening. Naturally, concerned individual such as myself, who having deeply entrenched symptoms, was referred by doctor to undergo further PSA tests on 20-06-08, 13-01-09 and 28-10-09.

    Yet following my latest annual check-up, encompassing all-inclusive pathology tests collection on 28-10-09, all-of-a-sudden having received QML Invoice No: 43060749 for the non-rebatable PSA (Ref No: 5922341), Item No: PB265. Listed with the deceitful annotation to the effect that the: “Current Medicare parameters do not allow a rebate for this test”! In other words on me was to pay $42.00 by the due date 28-11-09. If not for the fact that QML always bulk-billed Medicare on my behalf, and still continues to bill Medicare directly, except for the singled-out PSA test on this occasion. Surprisingly, I never received or having ever noticed any such cautioning info anywhere, concerning Medicare bulk-billing changes, affecting PSA tests rebate.

    Clearly flabbergasted, I went on 05-11-09 to (Myer Centre) Medicare Office in Brisbane, just to be told most outrageously that purportedly: “PSA screening charges were never rebated by Medicare”! Even though I declared on having previous PSA tests bulk-billed directly to Medicare, the wilfully ignorant attendant didn’t take notice of my testimony. Only when having asked for the supervisor, she went to return with the U-turn clarification. According to which, persevering citizens ultimately learn to know that: “Medicare rebated a single, annual PSA test”. If not for the fact that such a ruse contradicted my rebated PSA test on 13-01-09, in spite of having previously rebated PSA test on 20-06-08, and likewise earlier rebated PSA test on 14-11-07, undertaken at the six month intervals.

    Surprisingly, it wasn’t brought ever to my attention (and many other moral citizens), a vital call for adherence to the rebatable once a year PSA test. Instead the Rudd Government run Medicare rather to fleece disenfranchised men willy-nilly, extorting particularly senior citizens on the false pretences. Otherwise I have no qualms in regard of the single annual rebate for the routine PSA screening. So long, one being told apropos. It’s just a commonsense notion to be well informed by the government in the authentic democracy.

    However in my case (reflecting thousands, possible millions duped citizens), I was never informed about it. So as a consequence having undertaken 11 weeks prematurely my latest PSA test. Actually, I wasn’t even told by my GP about the ordered PSA test, in fact included within the general bundle of the annual pathology tests. Only subsequently, when I saw my GP on 09-11-09, he clarified to me that the decision came as a result of the ultrasound scan report (belatedly handed by me to doctor). Very timely indeed to detect rapidly doubling PSA figures. Consequently I was told by my GP to undergo next PSA test in six months (doctor’s reminder will be send to me).

    Yet just imagine when I presented my GP’s letter to the Medicare supervisor Louise on 10-11-09, she disregarded off-hand qualified doctor’s opinion: “Mr Braun was required to do a PSA assessment earlier than 1 year from the last PSA test as a necessary clinical follow-up to an abnormal finding on a recent U/S scan of the genito-urinary tract”. Actually, Ms Louise insinuated at one stage that extra rebates were previously granted, subject to doctor’s request for the essential PSA test. Yet about face she persisted in turn to delude me: “Medicare policy in regard of the PSA test rebates never changed”. According to which: “Only prostate cancer diagnosed patients to receive up to four rebates per annum for PSA tests”!

    Although, when I logically asked her: How such a clinical diagnosis can be achieved in the first place, without undertaking the most elementary PSA tests? She lashed back: “I’m not medically qualified to respond in regard”. What a devious cop-out, when apparently was widely known already that: “Early detection of aggressive prostate cancer through PSA testing may allow the disease to be treated early enough, and as such it can be cured. In fact, PSA levels may be elevated 5-10 years before prostate cancer produces any symptoms. Early detection may also reduce the risk of complications due to the treatment of the prostate, or of the disease itself. This in turn will lead to improved quality of life for men”!

    Sadly, deviously indoctrinated Medicare supervisor, empowered to browbeat humble men, wouldn’t care what a virtual medical centre.com offered online. Even what medicare australia.gov stated about the rebatable PSA screening changes. Which as a curtsey gesture should have been clearly conveyed to all previously PSA tested men. Who expected duty of care from the Rudd Government run Medicare, yet instead its hostile supervisor Louise went ballistic to throw at me that purportedly: it was on me to ascertain each time GP ordered pathology tests, if any of these intricately termed individual tests (often indecipherable) were no longer bulk-billed. Or else to cop the consequences of the prima-facie extortion. Because, according to Ms Louise, there was nothing in the Act to preclude the pathology service providers from changing billing practices willy-nilly, and thus ambush duped citizens of the smart state.

    Obviously without any prior notice as norm! Analogous to the removal “No Limit Parking” sign (along with its post) by the city council, whose inspectors to book in turn unwary motorists. Just as in “good old days”, when often invoked “the ignorance is not an excuse” motto, used to muzzle ripped-off motorists. Conceivably Ms Louise went to same finishing school to insist furthermore that the healthcare providers conglomerate, encompassing general practitioners and medical specialists could alter willy-nilly their billing practices without any prior notice to the bulk-billing patients. Who on the way out to discover that instead of the usual signing on the doted line, all-of-a-sudden being compelled to pay. Uttered provocative Ms Louise forty minutes down the track of the incessant bully tactics.

    Executed with impunity, still having chutzpah to point-out under reverse psychology ploy, that I could complain to minister. Seconded by QML, virtual monopoly of the smart state! Infamous apparently for its Call Centre, staffed by the deviously indoctrinated operatives. Who acted in unison with the trial by ordeal norms. Undoubtedly smart state usurped citizens (akin to others in the lucky country) would be stun to discover that the Rudd Government entrusted institutions with the pathology tests execution as an integral part in the healthcare delivery, colluded with the Medicare. Thus ultimately jeopardising PSA screening to the detriment Aussie men, such as myself. Who first called on 05-11-09, QML account enquiries: 1800 350 046, to no avail.

    Just to be told that QML was a private institution in business (to profiteer whatever it takes — without any responsibility to the community) which provided specialist laboratory analysis for me, as per GP’s order. Hence QML expected $42.00 by the due date 28-11-09. If not for the fact that QML always bulk-billed and still continues to bill Medicare directly, except for the singled-out PSA test on this occasion, Item No: PB265. Listed with the deceitful annotation to the effect that the: “Current Medicare parameters do not allow a rebate for this test”! Anyway, significant change occurred to the detriment bulk-billing clients, yet instead of informing those previously PSA tested, QML coerced for payment instead. While deliberately withholding vital information about the rebatable PSA test once a year.

    In no time would I ever notice any such cautioning advice display or informative pamphlets be given within the QML Pathology’s collection centre, concerning significant change, affecting Medicare rebate for PSA test. Even courteous QML Pathology’s collection centre staff, when asked about the change in PSA test allowance (affecting bulk-billing patients), none had a slightest idea about. “Because they didn’t receive training on the matter in question”, responded Simon to that effect, when I called QML account enquiries on 10-11-09. Contacted crucially to find-out where should be send my GP’s statement. Confirming specific doctor’s request for the warranted PSA test, undertaken earlier than 1 year from the last. As a necessary clinical follow-up to an abnormal finding.

    Yet QML Call Centre’s Simon rebuffed immediately off-hand my doctor’s opinion, in a cavalier fashion. While declaring that: “Only prostate cancer diagnosed patients to receive extra rebates (up to four per annum) for PSA tests”! Utterly absurd contention at the time when prostatehealth.org insinuated a bit better: “Men who haven’t been diagnosed with a prostate condition can receive a Medicare rebate for a PSA test only once every 12 months”! Anyway, when Simon was asked: How such a clinical diagnosis can be achieved in the first place, without undergoing the most elementary PSA tests? He diverted on the wild goose chase, thus compelling on me to ask for the supervisor. Only to score yet an another rude, call centre mouthpiece, who very reluctantly gave her (Filia) name. While passing a buck to Mark Ward who she insisted was the right person to address my complaint (in writing).

    Finally, sick and tired of a buck being passed around while sustaining terrible ordeal and utter humiliation along the way (accomplished by the ruthless operators who had me for a punching-bag diversion), having finally recaptured in writing the ongoing saga. An eye opener to many, still failing to realise how vulnerable we are. Due to the absence essential Charter of Rights & Obligations in the commandeered nation. One wonders if the Court of Law to uphold the miscarriage of justice?     When my doctor who issued his clarification on 09-11-09, still believes that there is a significant difference between the routine annual PSA screening, and the distinctly specific doctor requested PSA test, undertaken earlier than 1 year from the last.     Last but not least QML’s Call Centre’s Simon insinuated that any of the numerous blood tests rebated currently, could change all-of-a-sudden without any prior notice. While adding, that I had too good for too long, benefiting from the rebated pathology tests.
