    $2000/day is about the going rate for locum specialists too. These remote GP positions are really emergency department positions in remote areas: very busy and lot of responsibility so it is entirely fair they get the same rate as specialists. The hours and isolation from family means no-one (at least, no-one sane enough that you would actually what to see them as a patient) could do this for a whole year and earn your hypothetical figure of $620k.
